After taking a look at your screenshot you attached (thanks for that by the way), things do look a little bit harsh.
You can clearly see packet loss starting at hop #2, and carrying through to the final destination. This would indicate that there is some sort of issue somewhere between hops 1 (which I believe is your modem) and hop 2, which looks like it *may* be a device outside of your home network.
